accountable, traceable, and operationally valid from the initial escalation
trigger until the escalation process has been formally resolved and closed.

The module establishes the foundational conditions required to ensure that
escalation is not treated as a single event but as a continuously governed
lifecycle with no uncontrolled interruptions, abandoned responsibilities, or
unresolved governance states.
An escalation begins with a trigger.
An escalation ends only when governance formally resolves it.
